What You Will Do:

• Provide expert intermediate care in a community therapy setting to support patient recovery and independence.
• Manage complex moving and handling cases, ensuring safety and adherence to best practice.
• Deliver tailored rehabilitation interventions, working closely with patients and families to create personalized care plans.
• Maintain compliance withHCPCstandards, ensuring professional and ethical practice.
• Leverage your community experience to engage effectively with local health services and drive positive outcomes.

Requirements:

• HCPC registration as an Occupational Therapist.
• Minimum of 2 years'experience in community and rehabilitation settings.
• Car driver preferable for travel between multiple sites.
