An exciting opportunity has arisen for aBand 6 Occupational Therapistto join theCommunity Rehabilitation Teamin Basildon. This full-time locum role offers up to£27 per hourfor an initial3-month contract. You will play a vital role in delivering rehabilitation services to vulnerable patients within the community, supporting individuals to regain independence and improve their quality of life following discharge.

What you will do:

• Conduct comprehensive occupational therapy assessments within community settings, including care homes and patients'own homes.
• Identify functional needs and establish personalised rehabilitation goals for patients on the Discharge to Assess pathway.
• Develop, implement, and review effective rehabilitation and care plans.
• Work collaboratively with multidisciplinary teams to ensure coordinated patient care.
• Monitor patient progress and adapt interventions to achieve the best possible outcomes.
• Provide advice, guidance, and support to patients and families to encourage engagement in rehabilitation programmes.
• Maintain accurate and up-to-date clinical documentation in line with professional standards.

Requirements:

• HCPC registered Occupational Therapist.
• Previous community rehabilitation experience is desirable.
• Ability to work independently within community settings.
• Full UK driving licence and access to transport required for community visits.
