What you will do:

- Conduct holistic, strengths-based assessments of adults with care and support needs, ensuring their voices are heard
- Develop, implement, and review personalised care and support plans to promote independence and wellbeing
- Engage in safeguarding enquiries as required under Section 42 of the Care Act, always prioritising the safety of individuals
- Collaborate with individuals, families, and carers to foster independence and improve quality of life
- Manage a diverse caseload, addressing complex cases involving risk, mental capacity, and safeguarding issues
- Apply relevant legislation including the Mental Capacity Act 2005 and Human Rights Act 1998 in your daily work
- Partner with multi-agency teams including health, housing, and the voluntary sector to ensure comprehensive support
- Maintain accurate and timely case records, always striving to meet and exceed organisational standards
