Review Assessment Support Officer, Thornton-Cleveleys

We have a role come up for a Review Assessment Support Officer working with Lancashire Council. The role is to contribute towards an initial screening and contact and overview assessment service under the direction of the Team Manager / Social Worker and to commission services as appropriate. This is a temporary assignment for 3 months with the potenatial to become a long-term post. Responsibilities: * To liaise as necessary with relevant staff in Contracts section about service provider issues. * To attend and participate in meetings as required * To contribute to the planning of future services by contributing to the analysis of Care Planning and purchasing information and to advise the manager of any perceived improvements * To contribute to the assessment process for equipment/minor adaptations as part of the Single Assessment Process under the guidance of appropriate professionals. * To contribute to the initial reviews of assessments and make amendments to care plans/commissioned care plans as necessary, under the guidance of the Team Manager / Social Worker. * To undertake scheduled and unscheduled reviews of current service provision to individual clients, to check the effectiveness of the Care Plan in meeting identified needs, to check the quality and effectiveness of the provider and to review the needs of the service user, under the guidance of the Team Manager / Social Worker Location: Thornton Cleveleys, FY5 5EB Hourly rate: £10.79 If this position is of interest, please contact Jake Singh today at Belmont Recruitment on (phone number removed)
