You will work closely withservice users, families, community organisations, reablement services, and commissioned providersto ensure the best possible outcomes.

What You Will Do

• Conductcomprehensive strengths-based assessmentsof adults to identify needs, risks, and opportunities to promote independence.
• Work collaboratively withcommunity and voluntary groups, reablement services, and commissioned providersto create effective support plans.
• Use apreventative and solution-focused approachto reduce reliance on long-term care services.
• Communicate effectively withservice users, families, carers, and professionalsto ensure coordinated and person-centred support.
• Support individuals to accesscommunity resources and informal support networks.
• Maintainaccurate, timely records and documentationin line with local authority requirements.
• Contribute toservice improvement and efficient flow through Adult Social Care pathways.

Requirements
Essential

• Experience working inAdult Social Care, ideally in assessment or front-door services.
• Strong understanding ofstrengths-based practice, prevention, and community-focused support.
• Excellentcommunication, decision-making, and problem-solving skills.
• Ability to work both independently and as part of amultidisciplinary team.
