PCN Clinical Pharmacist - General Practice, Bradford

The post holder will demonstrate substantial experience in primary care, including the effective use of both SystmOne. They must be a competent independent prescriber and have successfully completed the CPPE Pathway. Experience of leading a team and active involvement in training is essential.

The post holder will be a qualified independent prescriber who has completed their CPPE training.

Job duties will include:

- Audit Work including MHRA alert and Qof.

- Acute prescription requests

- Medicines reconciliation from a clinic/discharge letter

- Medication review

- Structured Medication Reviews

- Hypertension reviews – including adjusting doses/initiating new meds

- Contraception Reviews

- HRT reviews

- Depression reviews/Mental Health reviews – including weaning patients off

anti-depressants

- Opioid reviews

- Benzodiazepine reviews

- DOAC reviews

- Statin initiation and QRisk interpretation

- Thyroid dose reviews

SystmOne experience is required.
