Role Overview

We are seeking a qualified and experienced Social Worker to join our County Wide Hospital Discharge Team. This is a permanent post covering Chesterfield Hospital Discharge Team, with potential to cover wider services as demand arises.

Key Responsibilities

The team primarily provides short‑term assessment and support to maximise independence and prevent unnecessary admission, enabling people to continue living in their own home as long as possible. The team works within tight discharge timescales and will appeal to applicants who thrive in a busy integrated multi‑disciplinary setting.

You will be part of a supportive multidisciplinary team focused on improving outcomes for people discharged from hospital. The role also includes working at the Front Door to support Admission Avoidance, helping individuals remain safely in the community wherever possible.

Required Qualifications and Experience

Applicants must be qualified and experienced as a Social Worker, and due to the post involving children and/or vulnerable adults, a Disclosure and Barring Service check is required. Criminal record will not automatically disqualify candidates; each case is judged individually.

Working Hours and Pay

Unsocial hours, including weekends and bank holidays, may be required. Saturday and Sunday hours are paid at plain time unless between 7 pm and 7 am, when enhanced rates apply. Enhanced pay rates are paid for bank holiday working.
