As a Social Worker in the Adults Frontline Team, your expertise will be critical in undertaking assessments, reviews, and support planning under the Care Act, with a focus on technology-enabled solutions. You will also conduct assessments under the Mental Health Act and associated legislation, working collaboratively with health and other partners to deliver effective, least restrictive, and person-centred services to enhance customer outcomes.

- Undertake comprehensive and insightful assessments, reviews, and support planning to deliver tailored solutions under the Care Act.
- Embrace and integrate new technologies to provide efficient and impactful support planning.
- Conduct thorough assessments under the Mental Health Act, ensuring all services meet legal requirements.
- Collaborate closely with health professionals and other partners to create and execute effective, least restrictive, person-centred care plans.
- Advocate for adults within the community by developing and implementing service strategies to enhance their quality of life.
