Description
to work in the Enhanced Hospital team (EHT) supporting discharges from hospital under the Discharge to Assess process, setting up residential/domiciliary packages or step down provisions ensuring the Local Authority's compliance with the relevant legislation - Care Act 2014 Mental capacity Act 2005, Mental health Act 1983 etc. To work with and liaise with relevant teams and services such as the LA Community Wellbeing Teams. This team works a 37 hour contract but over a 7 day period
