What you will do:

• Acute Assessments:Complete comprehensive physiotherapy assessments within an acute hospital setting.
• Treatment Planning:Develop and implement tailored treatment programmes to meet individual patient needs.
• Multidisciplinary Working:Collaborate closely with consultants, nurses, occupational therapists, and other allied health professionals.
• Caseload Management:Prioritise and manage a busy caseload effectively based on clinical urgency.
• Discharge Planning:Support safe and timely discharges, including onward referrals to community services.
• Clinical Documentation:Maintain accurate, clear, and up-to-date patient records.
• Patient Education:Provide advice and education to patients and families to support recovery and independence.
• Continuous Development:Engage in CPD to stay aligned with best practice and current evidence.

Responsibilities and general duties:

• Experience:PreviousBand 6 experiencewith recent work inacute medical settings, includingageing health and frailty.
• Discharge Expertise:Strong skills in discharge planning and patient flow management.
• Registration:HCPC registrationis essential.
• Minimum Experience:At least2 years'relevant post-qualification experience.
• Driving:Being acar driveris preferable to support travel across sites if required.
