What you will do:

• Conduct strengths-based Care Act assessments and create support plans tailored to individual needs.
• Coordinate hospital discharge planning and manage Discharge to Assess pathways efficiently.
• Collaborate closely with health colleagues to facilitate safe and timely patient discharges.
• Engage in multi-agency work across health, housing, and community services to provide well-rounded support.
• Safeguard adults, manage risks, and ensure the protection of vulnerable individuals.
• Promote independence, recovery, and person-centred outcomes for all service users.
• Maintain and manage an active caseload, ensuring accurate records using Liquid Logic.
