Nurse Specialist Case Manager, East Sussex

On behalf of my client we are recruiting for a Nurse Specialist Case Manager for the East Sussex area. You must be a professional person with excellent communication skills, a forward thinking aptitude, confident yet able to empathise. Quality and safety without being risk adverse at the forefront of your mind with the passionate drive to promote and bring the specialist team forward. The ideal post holder would be an experienced post holder within the Social Care sector with a good understanding of managing complex health conditions at home. You must be a RN with current NMC registration - Adult Nursing. Have a minimum of two years post registration experience in an acute clinical environment. A proven track record of delivering high quality customer service. You must have an understanding of Continuing Health care funding and be a Post holder within Social Care Sector and have previous management role. You will responsible for the safe and effective high quality delivery of care for the specialist care customers. You will complete all customers clinical assessments and subsequent paper work. Complete bespoke protocols for all nurse led and nurse managed tasks. Oversee all remaining paperwork work ensuring it is up to date and accurate. Review all customers and their care plans and all associated paperwork. To ensure all customers paper work in the home and in the office is fully compliant at all times and ready for an unannounced internal audit or CQC inspection. Undertake the HCA two day training, mark papers and assess if competent. Compete one to one training observations with HCA's and nurses for all nurse led tasks. This is a full time position working Monday to Friday 9am - 5PM. The salary is £30,000 - £35,000
