What You Will Do:

• Conduct comprehensiveCare Act assessmentsfor adults with learning disabilities and develop personalised care and support plans.
• Collaborate with professionals, partner agencies, and families to deliver holistic, outcome-focused support.
• Promoteindependence, inclusion, and community accessfor service users.
• Monitor and review care plans, adapting support in response to changing needs.
• Maintain accurate and compliant case records in line with legislation, policy, and best practice.
