What you will do:

- Conduct Care Act Review assessments primarily within North East Lincolnshire, including Grimsby, Cleethorpes, and Immingham
- Collaborate with individuals, families, and relevant agencies to deliver comprehensive support plans
- Focus on community-based service delivery, ensuring that assessments are focused on person-centred care in homes or residential settings
- Work closely with a dedicated team to ensure all care plans meet legal requirements and ethical standards

We are looking for one or possibly two qualified agency social workers to join us at Focus. These dynamic roles allow you to work primarily in the community, where your input will directly influence positive outcomes for adults in need of support. While remote work is possible, the role requires availability within North East Lincolnshire for home visits and assessments.
