Role overview

An exciting opportunity has arisen for an experiencedChildren's Social Workerto join theThrough Care TeaminHartlepool. This role focuses on supporting looked after young people aged 16+ and care leavers up to the age of 25, helping them navigate a successful transition into adulthood.

This is a rewarding and impactful position offering flexibility, professional challenge, and the chance to make a lasting difference in young people's lives. The post is initially for three months, with the strong possibility of extension.

What you will do

• Manage a caseload of looked after young people aged 16+ and care leavers up to 25.
• CompletePathway Plansand conduct regular reviews focusing on education, employment, accommodation, health, and emotional wellbeing.
• Support young people to build independence, resilience, and informed decision-making skills.
• Safeguard young people in line with statutory responsibilities, identifying and managing risks such as homelessness, exploitation, mental health needs, and substance misuse.
• Work collaboratively with multi-agency partners to secure accommodation, education, training, and employment opportunities.
• Build consistent, trusting relationships usingstrengths-based and trauma-informed approaches.
• Advocate for young people's rights, aspirations, and long-term outcomes.
• Maintain accurate, timely, and high-quality case records and reports.
• Participate in supervision, training, and service development activities to support continuous improvement.
