You will play a key role in supporting adults with learning disabilities and complex needs, promoting independence, wellbeing, and positive life outcomes through strength-based and person-centred practice.
Key Responsibilities
Assessment&Care Planning
- Undertake comprehensive strengths-based assessments in line with the Care Act 2014.
- Develop, review, and manage person-centred care and support plans for adults with learning disabilities.
- Complete Mental Capacity Assessments and make Best Interests decisions in accordance with the Mental Capacity Act 2005.
- Prepare reports for the Court of Protection where required, ensuring full legislative compliance.
- Lead and participate in safeguarding enquiries under Section 42 of the Care Act.
- Promote independence while effectively assessing and managing risk.
- Work collaboratively with health professionals, housing services, and other partner agencies to deliver coordinated care.
- Support smooth transitions from Children's Services to Adult Services, enabling access to employment, education, and community resources.
- Manage a varied caseload, maintaining accurate and up-to-date records in line with statutory requirements and council policies.
