Job summary
This post is fixed term / secondment until 31.03.2027 to meet the needs of the service.

An exciting opportunity has arisen to join the West Community Services Team as a Clinical Care Co-ordinator for the Caernarfon / Dolgellau / Amlwch/Benllech CRT on a fixed term basis. The successful candidate will support the CRT through referral triage, co-ordinating rapid response, and discharge follow up with the aim of providing the right care, at the right time, in the right place and by the right person. This role involves being an integral member of the Multi-Disciplinary Team and will require close working with all staff to ensure delivery of targets and providing a quality and safe service for patients, relatives and staff. The post holder should have excellent communication skills, be an organised and pro-active individual as well as being motivated and possessing good leadership skills. Main duties of the job
The Caseload Holder will manage a defined caseload in line with good practice with responsibility over a 24 hour period.
The post holder will be involved in the complex needs assessment i.e. CHC, Health and Social Care, unwell patients.
The post holder will be expected to work collaboratively with other agencies for the provision of patient services without direct supervision.
The post holder will be involved and expected to undertake a range of management duties and deputise in the absence of the Community Resource Team Leader.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
