Role overview

An exciting opportunity has arisen for an experiencedSocial Workerto join theAdult Community Team in Hullon an ongoing locum basis. This role offers flexibility across part-time and full-time hours, with a hybrid working arrangement requiring a minimum of three days per week in the office. Based at the Orchard Centre, you will have the opportunity to negotiate working patterns with your manager while delivering high-quality statutory social work services across community settings.

You will play a key role in completing statutory reviews, supporting adults to achieve positive outcomes, and ensuring care packages and direct payments are meeting assessed needs.

What you will do

• Complete statutory reviews across a range of settings, including care homes, community environments, and day services.
• Assess and review care and support plans to ensure outcomes are being met effectively.
• Monitor and review direct payments to confirm they are appropriate and compliant with requirements.
• Work collaboratively with health professionals, providers, and partner agencies to deliver joined-up care.
• Maintain accurate, timely, and high-quality case records in line with statutory and organisational standards.
• Provide advice, guidance, and support to adults, families, and carers, promoting choice, independence, and empowerment.
