This is an exciting opportunity for an experiencedSocial Workerto join theHome First Hospital Teamin the stunning region ofEast Lothian. Based acrossJohn Muir House,East Lothian Community Hospital, and occasionally theRoyal Infirmary of Edinburgh, this role sits within afast-paced Unscheduled Care service, embracing theHome First approachto support safe, timely discharges and prevent unnecessary hospital admissions.

You'll be working in adynamic, multidisciplinary environmentwhere your professional judgement, adaptability, and commitment to person-centred care will be highly valued.

What you will do:

• Respond promptly to referralsvia daily multidisciplinary team meetings
• Deliver short-term, focused interventionsto stabilise situations and support safe outcomes
• Work collaboratively with MDT colleaguesto prevent unnecessary acute hospital admissions
• Support A&E turnaround, assisting patients to avoid ward admission where appropriate
• Apply strong decision-making skillsin a fast-moving, unscheduled care setting
• Travel between sites across East Lothian, using your car and driving licence to provide timely support
• Utilise MOSAIC experiencewhere applicable (desirable but not essential)
