What you will do:
  • Manage and prioritise a diverse caseload, delivering personalised care for adults with complex conditions such as frailty, dementia, and neurological issues.
  • Collaborate with the multidisciplinary team and oversee the delegation to non-registered workforce to ensure holistic patient care.
  • Undertake holistic assessments, including pressure management, and address nutrition and hydration needs.
  • Ensure safe and efficient hospital discharges and prevent admissions by providing timely and effective interventions.
  • Utilise your knowledge of equipment provision, aids, and adaptations to improve patients'daily living activities.
  • Offer guidance and support on Mental Capacity Act awareness and unwise decisions.
