My client is one of the UK?s premier providers of support services for people with diverse and often complex needs in community settings. They provide specialist support services for people with learning disabilities, mental health needs, autism, profound and multiple learning disabilities, sensory and communication impairments, and acquired brain injuries. Job Purpose Generate new referrals/business for the company, working with adults with learning, physical, complex, mental health, brain injuries and residential needs who are looking to access supported services Main Responsibilities Conduct person centred needs and risk assessments following referrals Prepare support proposals based upon assessment of need Develop person-centred services through the production of comprehensive and high quality support plans and associated documentation Work with Business Development Managers to source and agree property as necessary through housing partners and private landlord routes Work effectively with internal operational colleagues, quality, health and safety and PBS teams and external agencies to prepare a project Implementation plan for new service development and user transition if necessary Organize the recruitment and training of staff as services are secured and brought on-line Liaise with the individual and their health professionals i.e. case managers, social workers, commissioners, consultants, occupational therapists, speech & language, advocates, family members, other care providers and rehabilitation teams in order to encapsulate each individuals health/support needs/requirements Complete initial care provider assessment and propose funding packages to the relevant social care provider which enables the individual to live independently in the community with their required support needs Actively market the company to social care authorities and support their business development growth Essential Experience/Requirements Full clean driving licence Experience working in an Adult?s Social Care Environment Demonstrable experience of working towards KPIs and exceeding targets
