Description
We are looking for an experienced Mental Health Social Worker, preferably AMHP. The post is based from home, with some essential travel to Shrewsbury. Our supportive team is a small hospital based discharge social care team and our main purpose is to ensure safe discharges and social care input if required. We also complete Care Act Assessments and Section 117 discharge meetings. The post is for an initial term of three months, which may be extended
