Qualified Social Worker - Adults Hospital Discharge Teams, SE1 7EH

We are currently looking for a number of Social Workers who have qualified experience of working across Adults Social Care. Specifically in Hospital Discharge Teams! There are multiple Hospitals across London which are currently looking for new staff to help keep the service running smoothly and continuously improvising during these tough times. Vacancies are mainly full time as a locum social worker. This will be between 35-37 hours per week with an on-going contract. Duties Include: * Facilitating safe and timely hospital discharges along with arranging support and plans for package of care. * Undertaking Assessment of needs, Mental Capacity Assessment, Continuing Care Assessments * Undertaking care and support plans for long term package of care and placements within residential and nursing homes * Undertaking Adults Safeguarding enquiries * Completing NHS checklists. * Taking part in duty work You might be looking for somewhere closer to home, a step up in your career, a change in team or even a pay increase?
