Case Manager, Welwyn Garden City

Case Manager Location: AL8 7SR Salary: (£30-34k per-annum pro-rata) Working Hours: Full Time - 5 days per week plus on call duties every 4 weeks As a Case Manager, you will ultimately be responsible for managing a team of carers to deliver outstanding live-in care for our clients. You will be a direct case manager for a geographically defined pool of clients with varying needs. You will lead the team of carers to ensure they are trained, motivated and inspired to deliver outstanding live-in care. As an advocate for high-quality care and the ENA values, you will also support the business to grow by starting new care packages, establishing a routine and foundations for a plan of care. As part of the case management, you will be a team-player and partake in a one in four on-call rota to support our clients consistently and provide a good level of continuity. Case Manager Requirements: * A minimum of two years experience as a Qualified Nurse, Occupational Therapist or Physiotherapist; * You will be able to and enjoy problem solving, demonstrating adaptability at all times; * Tenacious and passionate about delivering high quality care; * An excellent communicator with strong people management skills; * Driving Essential; Case Manager Benefits: * Competitive Salary (£30-34k per-annum pro-rata); * 28 days paid leave per annum (pro-rata), with increasing entitlement for each additional year of service; * Potential flexibility to work remotely; * Bupa Cash-plan; * Defined Pension Scheme; * Ongoing support, training, and professional development; Case Manager Role and Responsibilities: * To manage an assigned caseload, covering and supporting other team member?s caseloads to ensure that all our clients are given an outstanding service; * Keep detailed, timely records alongside carrying out case management and auditing tasks to deliver the best care; * To be responsible for the management and support of high quality care services for designated clients, care staff and teams within ENA; * To act as an ambassador for the company, championing high quality person-centred care by building outstanding care plans that reflect the clients wishes and their need to exercise choice and independence; * To share your knowledge with your team and encourage professional reflection discussions within the team to improve the overall performance; * To ensure all care staff contribute to the best of their ability to the efficient operation of the business by providing them with leadership and supervision. Utilising performance management tools and techniques where necessary; * To build a rapport with external individuals and multidisciplinary teams. Communicating with a diverse range of internal and external contacts; * To contribute to the social environment that we have worked hard to establish.
