Key Responsibilities
- Assess adult care and support needs and develop detailed, outcome-focused care plans
- Implement and regularly review support plans to ensure needs continue to be met
- Undertake adult safeguarding enquiries and contribute to safeguarding plans
- Provide guidance and advice to colleagues and partner agencies on safeguarding matters
- Promote independence using recognised social work methods, models and tools
- Make sound professional decisions in line with current legislation and policy frameworks
- Work collaboratively with service users, families and partner organisations
