What you will do:

- Conduct assessments, interventions, and provide follow-up support for patients with dementia in community and clinic settings.
- Collaborate closely with clients, key workers, and multidisciplinary teams to maintain a high standard of personalised client care.
- Serve as care coordinator, ensuring clients'needs are met through a structured and reviewed care plan.
- Administer medications in compliance with policy, while providing necessary education and support to clients and carers.
- Liaise with Consultant Psychiatrists and GPs for patient assessments and follow-up related to anti-dementia medication.
- Participate in the development and evaluation of specialist dementia care practice and facilitate case conferences as needed.
- Contribute to policy development and the sharing of best practices in dementia care.
