Ongoing Support The Ongoing Support Teams are locality based and offer social work and occupational therapy services to those who require longer term social care support. The teams help those most in need by listening and responding to the specific needs of individuals. They work alongside people to identify what is important to them and support our customers to make a plan to improve things that aren?t going so well so that they can live the life they want to. We work with health partners to help people before they reach crisis point. We focus on identifying existing individual, family and community support, ensuring that people can live independently for as long as possible. Rather than assuming responsibility for providing care, we look for local, sustainable and less intrusive solutions wherever they are available. We help people access the wide range of opportunities for intervention and prevention that are provided. We are looking for both Level 2 and Level 3 Social Workers to join our teams in Trowbridge and Chippenham. About the role of Experienced Social Worker * Work with individuals and communities to promote resilience, independence and wellbeing * Be a formal assessor for newly qualified social workers undertaking Assessed and Supported Year in Employment (ASYE) * Manage a caseload of work using person centred practice principles to assess, support plan and review with the aim of promoting well-being and positive risk taking.
