Social Worker, Midsomer Norton

Bath and North East Somerset Council are looking to recruit a fully qualified Social Worker (Level 2) to join our Adult Social Care continuing health care service (CHC).

This exciting, specialist position will play a vital role in identifying, across all adult care service user groups, people with health care needs that may be above those which can be lawfully met through the Care Act 2014. With the support and guidance of the CHC Team Manager you will carry out CHC assessments alongside a CHC Nurse Assessor and make joint recommendations regarding each person’s eligibility for full NHS funding. The identification of people whose needs could be most appropriately met through joint health and social care funding is also required.

This role will suit someone who has successfully completed their ASYE and ideally has some knowledge and experience of what continuing health care entails. A full induction programme will be provided alongside ongoing professional development opportunities.
