The remit of the team is to undertake work within the Learning Disability and/or Autism Programme to support people to be discharged from long term hospital stays, that require specialist bespoke placements and who are subject to the Mental Health Act 1983; people may also be on forensic sections MHA 37/41 and subject to the Ministry of Justice. We also work with our commissioners, health colleagues and the ICB Learning Disability and/or Autism Programme in preventing hospital admission and supporting with community placement breakdowns.

As a valued member of this team, you will need to have a good understanding and experience of the application of the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983.

You will be working in an exciting and sometimes challenging environment in close partnership with external agencies and providers, which will require high levels of confidence and independence. The role will also require strong leadership skills and providing supervision for Social Workers in the team.

You will have experience of working creatively with adults who have care and support needs and will bring a commitment to the principles of strengths-based practice, promoting independence and wellbeing whilst safely managing risks. In return, you will receive plenty of support and training to help you reach your potential. We will provide regular formal and informal supervision due to the nature of the complex case work.

You will need to be a Qualified Social Worker who is a Practice Educator, BIA or AMHP qualified.

The team has adapted hybrid working, where you will be partly office and partly home-based. You will need to have effective communication, negotiation, organisational and IT skills, which are essential tools to aid developing flexible working practices.

You must be able to drive and have use of a vehicle for work purposes (a taxi service will be provided where disability precludes this) and registered with Social Work England.
