Assessment Care Co-Ordinator, Oxfordshire

An exciting opportunity to join an established Mental Health team as a Care Coordinator has become available with one of our clients. You will be joining the Adult Mental Health Team (AMHT) helping to provide a high quality of service to all service users in the areas of the Adult Mental Health Service. The candidate for this role will primarily be an assessment and duty worker for Bucks Early Intervention's assessment function. It will primarily be to triage assessments and undertake assessments. In line with the access and waiting time standards for first episode psychosis, working with individuals between the ages of 14-65. The successful candidate will also be allocated a small caseload to care coordinate. The service is county wide and work across Buckinghamshire. The role will mainly be office based but with some flexibility due to Covid restrictions. To apply for this role you will need to have a qualified with a social work/OT/Nursing qualification (Dip.SW, CCSW, CSS) or equivalent, 2 years post qualification experience, a current DBS and will need to be HCPC registered. Previous experience should include working in a community or mental health team. A driving licence and access to a car will be required.
