Responsibilities

To work as an integrated part of the multi‑professional team in the Living Well Service.
To plan, organise and facilitate a range of creative activities and craft projects within the Living Well Service unit, enhancing the patients and carers experience.
To ensure that specialist skills, creativity and experience are used to encourage patients and carers to find and express their own creativity.
To assist the Living Well team with a range of duties relating to patient care and service delivery.
To work alongside the MDT in providing assessment, intervention and rehabilitation to patients attending the Living Well Service.
To contribute to the development of Living Well Service.
To maintain stock of creative activity materials and therapy equipment.
To record patient information.
