CLINICAL PHARMACIST Slough Salary £50k + benefits Medical Staffing are searching for a Clinical Pharmacist in the Slough area. The client is looking for a candidate who is able to independently prescribe. Be the key member of the practice in ensuring prescribing is effective, safe and that medicines use is optimised for patients. Duties * Lead as independent Prescriber. * To undertake medication review clinics with people with multiple long-term conditions and on polypharmacy to improve patient outcomes in all member practices. Some reviews may need to be undertaken in the patient?s home. * Contribute to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues * To act as a point of contact in the practice for all medication and prescription-related queries from patients and health care professionals. * To facilitate good working relationships between the practice and Community Pharmacies and Hospital Pharmacists. * To undertake clinical audits to support the practice in following NICE and local guidance and pathways. * To take responsibility for staying up to date and your own development and attend training events as appropriate. * To undertake minor ailment clinics or triage. * To undertake medication audits. * To manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions.
