What You Will Do

• Assess, evaluate, and review service users'needs in the community.
• Develop and implementindividualised, person-centred treatment plansto promote independence and safety.
• Undertakecommunity visits(access to a vehicle with business insurance is required).
• Work closely with themultidisciplinary teamto ensure coordinated and effective care delivery.
• Provide education, advice, and support to service users and their families/carers.
• Maintain accurate and timely clinical documentation in line with professional standards.
• Keep up to date with best practice and evidence-based occupational therapy interventions.
