About the Role:

Based inSouth Gloucestershire, you'll play a vital role in promoting independence and deliveringperson-centred social carein line with theCare Act 2014. You'll conduct assessments, develop care plans, and coordinate safeguarding interventions to ensure adults receive the right support at the right time. Collaboration with health, housing, and voluntary partners will be central to achieving positive outcomes.
What You Will Do:

• Conductstrength-based assessmentsthat identify personal strengths, networks, and resources.
• Develop and reviewperson-centred care planspromoting independence and wellbeing.
• Undertakesafeguarding enquiries (Section 42)and manage risk in line with statutory frameworks.
• Providecase managementfor adults with complex needs, including hospital discharges and community-based support.
• Work collaboratively withNHS partners, housing teams, and voluntary organisationsfor integrated service delivery.
• Maintainaccurate, timely case recordson Mosaic and adhere to professional standards.
• Participate inteam meetings, reflective supervision, and contribute to service improvement initiatives.
• CompleteCarer's Assessmentsand facilitate access to appropriate support services.
• UpholdMaking Safeguarding Personalprinciples in all aspects of practice.

About You:

You are aqualified and registered Social Worker (SWE)with solid knowledge of theCare Act 2014,Mental Capacity Act 2005, and related legislation. You excel in assessment, analytical, and report-writing skills, with a passion for delivering strength-based, person-centred, and outcome-focused practice. Experience in community-based adult social care will be highly beneficial.
