Key Responsibilities

• CompleteCare Act 2014 assessments, reviews, and risk assessments for adults and carers.
• Develop and implementperson-centred care and support plansto achieve positive outcomes.
• Apply statutory frameworks such as theMental Capacity Act 2005andSafeguarding Adults procedures.
• Promote independence and wellbeing through astrengths-based approach.
• Work collaboratively withservice users, carers, health professionals, and community partners.
• Manage a varied caseload, maintaining accurate and timely electronic records.
• Participate inmulti-disciplinary meetings, best interest decisions, and safeguarding enquiries.
• Provideprofessional advice and guidanceto colleagues and partner agencies.
• Contribute toservice development, reflective practice, and continuous improvement.

Essential Requirements

• QualifiedSocial Workerwith registration toSocial Work England.
• Experience working withinAdult Social Careunder the Care Act 2014.
• Strong understanding ofsafeguarding proceduresandcapacity assessments.
• Excellent communication, organisational, and record-keeping skills.
