What you will do:
- Provide comprehensive mental health assessments for patients within the surgery and during home visits.
- Develop and implement personalised care plans, taking into consideration each patient's unique needs.
- Collaborate with multidisciplinary teams to ensure holistic care for all patients.
- Offer expert advice and support to patients and their families, helping them navigate mental health challenges.
- Monitor and review patient progress, adjusting care plans as needed to ensure the best outcomes.
- Maintain accurate and timely documentation according to NMC standards.

Requirements of the Community Psychiatric Nurse:
To succeed in this role, you must hold a degree or equivalent in Mental Health Nursing, have experience working as a Mental Health Nurse, and be registered with the NMC. These qualifications ensure you deliver the best possible care to our patients.
