Your Responsibilities

• Conduct Care Act-compliant assessments to determine support needs and strengths.
• Manage a diverse caseload, including safeguarding matters, promoting independence and well-being.
• Work collaboratively with families, carers, health professionals, and community organisations.
• Provide advice, address safeguarding concerns, and connect individuals with appropriate services.
• Apply a solution-focused approach to guide decision-making and care planning.
• Ensure accurate and timely documentation in line with statutory requirements and best practice.

Candidate Requirements

• SWE registration and a recognised Social Work qualification
• Minimum of two years'post-qualification experience
• Confidence in undertaking mental capacity assessments and safeguarding work
• A proactive, community-focused mindset with excellent interpersonal skills
