Support Worker - Leaving Care Warringotn £8.75 - £9.75 an hour Temp to Perm Your new company You will be joining a well-respected leaving care provider who support young people in need aged 16-19 as they transition into independent living. Your new role ·Delivering ASDAN programme ·Completing assessments of needs of clients ·Support and monitoring Maintaining electronic records of the clients? needs and progress ·Working across multi-disciplinary services i.e. social workers, probation, healthcare ·You will work both within a team and in lone-working environments. ·The needs of the young people will vary but will include those with mental health needs, learning disabilities, involvement in the youth justice system, unaccompanied asylum seekers.
