Warrington - Social Worker - Adults

An exciting opportunity has arisen for a Social Worker to join the Rapid Community Response Service in the delivery of person-centred strength based assessments and support. The post holder will assess the needs and eligibility of adults with care and support needs within a rapid response intervention and care model of working. The service will work with people in crisis as a result of either a deterioration in their physical health or social circumstances where they may be at risk of being hospitalised. The post-holder will be part of a multi-disciplinary team of health and social care professionals who will respond within two hours and work with the person and their family to provide the appropriate care, treatment and support they need to remain safe and independent at home for up to 72 hours. The work is fast, varied and challenging and as a result you will acquire a diversity of knowledge and practical skills. Full induction and training will be provided and support is readily available from team members and the line manager. Experience of working with older people and/or adults will be an advantage in this role. The successful applicant will be able to demonstrate excellent communication skills and must be flexible, reliable and committed to personal and service development.
