We are seekingtwo experienced Adult Social Workersto join theHospital Discharge Team (Transfer of Care Hub)at Warrington Hospital. This is a fast-paced, high-impact role where you will play a crucial part in supportingsafe, timely, and well-coordinated hospital discharges.
Working closely with health professionals and multidisciplinary partners, you'll assess social care needs, reduce delayed transfers of care, and ensure individuals leave hospital with the right support in place. This full-time role offers a hybrid working arrangement, with approximately three days per week based on-site.
