What You Will Do

• Assess and support individuals using clinical reasoning and reablement skills.
• Develop personalised reablement programmes to promote independence and positive outcomes.
• Independently manage a full-time caseload while contributing to the wider team.
• Demonstrate experience in social care reablement or intermediate care services.
• Comply with all HCPC requirements as a qualified Occupational Therapist.
• Travel locally by car to provide high-quality care in clients'homes and communities.
