Hospital Discharge Social Worker, Hampshire

Social worker needed within Hampshire County Council - based in QA hospital

Based at QA hospital and will be onsite with potential to remote work once a week.

Role will be primarily duty which will include, triaging, resolving issues in a fast paced environment, safeguarding, managing expectations, speaking with patients and their NOK, managing a task list.

Up to 38 per hour

Requirements as a Social Worker include:

• SWE Registration
• Eligible to work in the UK
• Drive and have access to a car
• Post-qualified experience
• Degree level or equivalent in Social Work
