Key Responsibilities:

• ConductCare Act Assessments,Mental Capacity Assessments, and determinebest interest outcomesin accordance with current legislation.
• CompleteCommunity Deprivation of Liberty Safeguards (DOLS)assessments, as well asContinuing Healthcare (CHC) checklistsandDecision Support Tools (DSTs).
• Operate from astrengths-based approach, ensuring practices remainleast restrictiveand person-centred.
• Triagize and prioritise referrals, maintaining a streamlined and responsive case management process.
• Collaborate confidently within amultidisciplinary team (MDT), working closely with health professionals, community partners, and internal colleagues.
