About the role

We are seeking a motivated and compassionateBand 6 Community Psychiatric Nurse (CPN)to join theCommunity Mental Health Team for Older People (CMHTOP)in Surrey. Job purpose

To deliver high-quality mental health nursing care to older adults with complex mental health needs, promoting recovery, maintaining independence, and enhancing overall wellbeing within a community setting.
Key responsibilities
  • Undertake comprehensivemental health, risk, and capacity assessmentsfor older adults.
  • Manage a definedcommunity caseload, acting asCare Coordinatorunder the Care Programme Approach (CPA).
  • Develop, implement, and regularly reviewperson-centred care plansin partnership with service users, carers, and professionals.
  • Deliverevidence-based nursing interventions, including medication management and physical health monitoring.
  • Administerdepot medicationand support treatment adherence.
  • Identify and managesafeguarding concernsin line with Trust and local authority procedures.
  • Work collaboratively within amultidisciplinary team, including psychiatrists, psychologists, social workers, and occupational therapists.
  • Liaise effectively withGPs, primary care services, voluntary agencies, and other stakeholders.
  • Participate in MDT meetings, case reviews, and discharge planning.
  • Maintain accurate, timely, and confidentialelectronic clinical records.
  • Ensure practice complies with theNMC Code, Trust policies, and relevant legislation.
  • Contribute toclinical audits, service evaluation, and quality improvement initiatives.
  • Provide guidance, supervision, and support tojunior staff and students, acting as a professional role model.
Person specification
Essential criteria
  • Registered Mental Health Nurse (RMN) withcurrent NMC registration.
  • Post-registration experience inmental health nursing, ideally within community services.
  • Knowledge of mental health conditions affectingolder adults, including dementia.
  • Experience inrisk assessment, care planning, and CPA processes.
  • Ability to manage complex caseloadsautonomously.
  • Excellentcommunication, interpersonal, and organisational skills.
  • Basic IT skills for electronic patient record systems.
  • Full UK driving licencewith access to a vehicle for community-based work.
Desirable criteria
  • Experience withinOlder Adult Mental Health services.
  • Knowledge of theMental Health Act, Mental Capacity Act, and safeguarding legislation.
  • Experience supervising junior staff or students.
  • Non-medical prescribing qualification or willingness to undertake training.
