What You Will Do:

• Conduct Comprehensive Assessments:Collaborate with patients and their families to deliver tailored assessments and create personalised care and support plans.
• Manage Risk and Discharge Planning:Identify potential risks, develop appropriate mitigation strategies, and facilitate safe discharge processes.
• Collaborate with Multi-Disciplinary Teams:Work closely with healthcare professionals and external agencies to deliver integrated and person-centred care.
• Ensure Legal Compliance:Adhere to relevant legislation, including the Care Act 2014, Mental Capacity Act, and other regulatory frameworks.
• Promote Independence:Advocate for patients by empowering them to maintain their independence and improve their quality of life through effective support planning.

Skills and Qualifications:

• Essential:Degree in Social Work with current Social Work England registration.
• Experience:Previous experience working in a hospital discharge team or adult social care setting.
• Knowledge:Strong understanding of the Care Act 2014, safeguarding procedures, and best practices in hospital discharge.
• Skills:Excellent communication, analytical, and report-writing skills, with the ability to manage complex caseloads.
