Social Worker - Transitions, Wolverhampton

Experienced Social Worker needed for Children's Transition Service. The post holder will play a key role in ensuring young people who may be eligible for Adult Social Care under the Care Act have an effective transition from Children's services. The role will include casework with young adults; life planning, as well as some developmental work. The postholder will be required to work within the legislative frameworks (Children and Adults) and agreed Local Authority procedures. To be considered, you need to be registered with Social Work England, have at least 1 year of experience working with children and families and ideally some safeguarding experience.
