What you will do:

• Assess the needs of children with disabilities and their families to ensure positive outcomes.
• Develop and implement tailored, person-centred care plans.
• Work collaboratively with multidisciplinary teams to deliver integrated support.
• Advocate for children and families, ensuring their voices are heard.
• Review and update care plans regularly to maintain effectiveness.
• Maintain accurate and compliant records in line with statutory requirements.

What we're looking for:

• Qualified Social Worker with relevant experience in children's services, ideally within disabilities.
• Strong assessment, care planning, and safeguarding skills.
• Ability to work effectively both independently and within a team.
• Excellent communication and organisational abilities.
