What you will do:

• Triage and manage the clinical caseload for housebound patients, ensuring their nutritional needs are met efficiently.
• Provide support and dietary plans for patients on Oral Nutritional Supplements (ONS).
• Deliver specialised care for Home Enteral Feeding (HEF) patients, developing customised nutrition plans.
• Travel between the Hillingdon and Ruislip bases, with the requirement to hold a valid driving licence and have access to a vehicle.
• Work collaboratively with other healthcare professionals to provide holistic patient care.
• Maintain accurate records and document patient progress in line with HCPC standards.
